Booth/Tent Sign Up Form

Agreement & Liability Waiver: By submitting this form, I acknowledge that our organization agrees to staff our booth from set-up through the end of the event and to abide by all event guidelines, including maintaining a family-friendly and respectful environment. We understand that we must provide our own tent, booth, chairs, and any other equipment and materials needed. We understand that Choices Pregnancy Care Center, the event organizers, and the venue are not responsible for any loss, damage, or injury that may occur during the event. Our organization assumes full responsibility for our booth space, including set-up, tear-down, and any materials or equipment used. Participation is at our own risk, and we release Choices Pregnancy Care Center and its affiliates from any liability related to our presence at the event.
